The Chronic Complex Social Services Specialist responsibilities include:

• Being a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the community.

• The Chronic Complex Social Service Specialist provides therapeutic interventions through assessment, coordination, referral, and interdisciplinary planning.

• They are knowledgeable about treatment, social and economic implications for individuals and families as it pertains to their age, stage of development and discharge needs.

• Possess a clear understanding of discharge and transition planning and linkage with community resources.

• Demonstrates skills in planning, organizing, and managing multiple functions and complex processes.

• This position helps facilitate and expedite discharging of our patients with complicated and/or complex discharge needs.

Position details:

• Location: Winter Haven Hospital - Winter Haven, Florida

• Status: Full time, 40 hours per week

• Shift: Monday - Friday 8:00 AM - 4:30 PM

• Weekends: none

• On Call: No

Education:

• Required Master's Degree of Social Work or Related Field

Experience:

• Required 2 years Social Work in an acute care or hospital setting
