Job Title: Field Mid-Level Providers (NP or PA)

Location: Syracuse, NY

Compensation: Competitive package ($100,000 - $145,000 yearly) based on location and experiences

Primary Responsibilities:

• M-F 8a-4p

• Conduct in-home physical exams and consultations, focusing on preventive care and education.

• Ensure patient safety and well-being as the top priority in all circumstances.

• Maintain proper documentation and paperwork, utilizing the EMR system MedNT.

• Collaborate with the clinical team and act medically smart in emergency situations, handing off cases when necessary.

• Provide supplemental and supportive medical care, emphasizing general medicine and patient education.

Qualifications:

• 3-5 years of experience as an NP or PA.

• Active NPI, Medicaid/Medicare, and NP License numbers.

• Ability to practice general medicine and educate patients effectively.

• Strong communication skills and the ability to blend into diverse communities.
