CORE DC Case Manager in Washington, District Of Columbia

CASE MANAGER - Short-Term Family Housing

Hours of Work: Monday - Friday 9:00am to 5:30pm (one evening 11:00am to 7:30pm)

Salary Range = $45K - $50K

The Case Manager is tasked with connecting clients to supportive community-based services that will help them achieve stable housing and meet their goals, including health and wellness services, mentoring and tutoring, enrichment programming for children, and any other appropriate services. In addition, the Case Manager will:

  • Meet with the clients as required to conduct an assessment of strengths and barriers to obtaining permanent housing; evaluate progress and assist clients in developing and meeting individual goals
  • Develop a Family Housing Stability Plan by utilizing diversion or rapid rehousing strategies.
  • Use community outreach best practices to facilitate meaningful, coordinated connections to needed services, resources, and supports.
  • Ensure clients have access to services, and that services are helping clients to meet goals; provide support to clients by monitoring and evaluating outcomes and revising goals and plans as needed.
  • Identify benefits or entitlements that may be available to the client; advocate on behalf of clients if they encounter obstacles in obtaining services or housing.
  • Maintain data collection and case records as required by applicable rules and regulations.
  • Facilitate the development of self-help skills, support systems, coping strategies, and increased hope and self-esteem that are central to the process.
  • Ensure clients adhere to the facilitys rules and regulations.
  • Facilitate a warm handoff for clients exiting STFH.
  • Other duties as assigned by the Program Director or designee.

NECESSARY SKILLS:

  • Experience providing case management services including developing an individual service plan for at-risk populations.
  • Familiarity with community resources and understanding of challenges facing homeless populations.
  • Strong organization and time management skills.
  • Ability to establish and maintain trusting relationships develop and maintain appropriate boundaries.
  • Must demonstrate responsiveness to cultural differences and a commitment to the value of cultural competency.

PERFORMANCE EVALUATION CRITERIA:

  • 100% engagement with clients to establish reasonable and attainable goals
  • 100% compliance with timely case notes including data completeness and accuracy
  • 95% of clients connected to services, resources and supports
  • 100% identification of benefits or entitlements available to clients
  • 90% facilitation in the development of self-help skills, support systems and coping mechanisms

QUALIFICATIONS:

  • Associates degree or two years of college, required. Bachelors degree in social work, human services, or a similar degree, is preferred.
  • Minimum of two (2) years of experience with homeless services programs or provision of social services needed in lieu of degree.
  • Both education and work experience can be substituted if an individual has received a Peer Case Management Certification through DHSs Peer Case Management Institute.
  • Familiarity with HMIS and CATCH, a plus.
  • Individuals with lived experience are encouraged to apply. Lived experience will be considered in lieu of work experience
