Your Responsibilities

As a Hospice RN Case Manager , you will:

  • Provide patient care across the continuum, managing hospice services with empathy and expertise.

  • Assess, plan, implement, and evaluate individualized care plans for patients.

  • Support patients and families through end-of-life care with compassion and dignity.

  • Collaborate with interdisciplinary teams to ensure seamless care delivery.

  • Maintain accurate documentation and manage multiple priorities effectively.

What You Bring

  • Education : Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate Nursing Program.

  • Licensure : Current Registered Nurse (RN) license in Oklahoma.

  • Experience :

  • Minimum of 1 year in a clinical care setting.

  • Hospice or home health experience (preferred).

  • Skills : Strong nursing skills, ability to manage priorities, and dedication to patient-centered care.

  • Transportation : Reliable transportation to perform job responsibilities.

  • Ability to sit, stand, bend, lift, and move intermittently, with the capability to lift 50-100 lbs.
