We are seeking a dedicated Licensed Practical Nurse (LPN) to join our home care team. As an LPN, you will provide skilled nursing care to patients in their homes, following individualized care plans developed by registered nurses and healthcare providers. You will play a crucial role in ensuring our clients receive exceptional care while promoting their health and well-being.
Responsibilities:
Provide direct nursing care to patients in their homes, including medication administration, wound care, and vital sign monitoring.
Assist with activities of daily living (ADLs) as needed.
Collaborate with RNs, physicians, and other healthcare professionals to implement care plans.
Educate patients and their families on disease management, medication, and home care procedures.
Maintain accurate and timely patient records and documentation.
Monitor patient progress and report any changes in condition to the healthcare team.
Ensure compliance with all healthcare regulations and company policies.
Qualifications:
Current LPN license in Maryland.
Minimum 2 years of nursing experience (home care experience preferred).
CPR and First Aid certification.
Strong clinical and critical-thinking skills.
Excellent communication and interpersonal skills.
Ability to work independently and manage time effectively.
Valid driver’s license and reliable transportation.
