Job Description:

The Scheduling Coordinator is responsible for ensuring that patient care is coordinated and managed appropriately. The Scheduling Coordinator is responsible for ensuring that care and services are delivered appropriately. Respects and promotes the mission, vision, and values of VNA Health.

Job Duties:

  • Receives case referrals.  Reviews available patient information, including disciplines required, to determine home care needs.  Makes Assignment According to patient needs.

  • Ensures that authorizations are received and requested as needed for new referrals.

  • Assigns patients appropriately for continuity of care; ensuring productivity standards are met.

  • Responsible for coordination of all patient visits.

  • Coordinates and manages the monthly schedule of nurses and home health aides.

  • Support intake coordinator; receiving and processing new referrals, triaging for skilled need and same day visits.

  • Initiates conferences with nurse manager or program director regarding any questions about individual eligibility for services.

  • Receives and triages phone calls from patients and refers all sources, as related to scheduling.

  • Complies with accepted professional standards and principles.

  • Has working knowledge of state, federal, local and accreditation regulations for the delivery of home health services.

  • Able to prioritize and facilitate interventions that effectively solve or prevent problems related to day to day operations

  • Actively participates in quality management program.

    Qualifications:

  • College degree preferred or equivalent experience.

  • Experience in handling phones, medical records and data entry in a medical environment preferred.

  • Must be able to work in a fast-paced environment with demonstrated ability to juggle effectively multiple competing tasks and demands with a positive “can-do” attitude.

  • Must have high level of interpersonal skills to handle sensitive and confidential situations.

  • Must have knowledge of and be highly proficient in a variety of computer software applications in word processing, spreadsheets, database and presentation software (MSWord, Excel, Access, PowerPoint) and Publisher. Ability to learn new software quickly.

  • Must have ability to adapt to changing situations proactively.

  • Must have strong data management, data analysis, and data presentation skills.

  • Excellent written/verbal and interpersonal communication skills.

  • Proficiency of basic math skills.

    Physical Qualifications:

  • Ability to lift 50 lbs

  • Ability to push/pull 50 lbs

  • Ability to stoop, bend, and squat

  • Ability to walk up one flight of stairs

  • Ability to walk one mile

  • Ability to sit for one hour, minimum

  • Visual acuity and audio acuity
