Pay Rate: $26.50 HourlyPer Diem-minimum commitment of 2 weekend shifts per month, and 2 holidays per year

GENERAL SCOPE: Performs both nursing and personal care as required by the needs of residents. Assists residents with medication management program and assures accurate and timely medical/nursing record keeping. Acts as liaison between resident assistants and RN Nurse Manager.

EDUCATION / QUALIFICATION: Graduate of an approved school of practical nursing with current New York State Practical Nursing license. Possess good organizational, leadership and communication skills. By law is responsible for their acts and those acts they delegate to others.

ESSENTIAL FUNCTIONS:

• Performs nursing assessments as required for the care of residents during the shift worked. Assists in evaluating care needs and any functional changes of residents and reports to RN Nurse Manager.

• Manages the Medication Administration System for all residents.

• Assist Enriched Living Manager with medical record keeping.

• Communicates resident’s condition by notifying Enriched Living Manager, and the physician when necessary and appropriate.

• All other duties as assigned.

EXPERIENCE: 3 to 5 years experience as a Long Term Care or Home Health Nurse.
