East Bay Community Action Program (EBCAP) is seeking a full time Nurse Care Manager for our Health Center at 100 Bullocks Point in East Providence, RI. We are seeking a highly professional and motivated individual who enjoys working with a multi-disciplinary team in a community health setting.

The successful candidate will provide comprehensive screenings, assessments, care coordination services, disease education and self-management support to specific patient populations. Nursing care is performed in both office-based and community-based settings, depending on the needs of the patient.

The successful candidate must have a minimum of an Associates Degree in Nursing or equivalent, and licensed as a Registered Nurse (RN) by the RI Department of Health. Two or more years of experience in a community health setting, acute care, or emergency/urgent care working with primary care providers to coordinate care and disease management required. Previous experience in an electronic medical record (EMR) preferred; however, training is provided in EBCAPs EMR which is NextGen.
