POSITION SUMMARY

An Outpatient RN works with an integrated team of medical professionals to provide superior patient and family centered therapeutic care, systematically and continuously collecting and assessing data related to patient health status to develop, evaluate and execute the plan of care. An Outpatient RN establishes goals and strategies for meeting care coordination goals across settings with the patient, family and/or other care providers.

ESSENTIAL RESPONSIBILITIES

  • Utilizes specialized knowledge and advanced critical thinking, combined with necessary clinical skills to provide high-quality care for patients via patient visits and phone.
  • Demonstrates knowledge in human growth and development with regard to the specific chronological age, developmental age, or social maturity level of the patient.
  • Take nursing calls as scheduled.
  • Contributes to always providing a safe, therapeutic environment and supports activities that promote the patients functional goals and the prevention of complications.
  • Evaluates the care needed for goal attainment by individual patients with appropriate communication and documentation.
  • Actively participates in team meetings and conferences as needed and disseminates up-to-date interdisciplinary communication regarding the patients status.
  • Demonstrates effective oral and written communication skills to develop a rapport with patients, their families, and other team members to ensure the fulfillment of requirements for legal documentation and reimbursement.
  • Maintains clinical competency by participating in training and continuing education programs necessary for credentialing or maintaining credentials within identified time frames.
  • Maintains the privacy and confidentiality of all patient information in compliance with all legal and H.I.P.P.A. requirements.
  • Executes EKG, Telemed, Suboxon UDS, SSRA, OPN, CM billable services, Injections/L.A.I. s
  • Executes best practice initiatives to maintain and improve patient satisfaction.
  • Performs other duties as assigned by Director of Nursing
