Summary:

The Patient Care Associate provides administrative and clinical support, and works under the direct supervision of a registered

nurse with whom they are regularly teamed to deliver optimal patient care through the use of the nursing processes.

Responsibilities: countabilities

• Communicates with patients, families, physicians, unit nursing personnel and all hospital personnel in an accurate and

courteous manner. This includes contract services staff and ancillary support services.

• Communicates pertinent patient care needs and information to the Registered Nurse and/or Charge Nurse in a timely manner.

• Reports to the nurse when further direction is needed regarding patient care assignment and accepts delegation of tasks assigned.

• Initiates and maintains open communication with team and nurse manager, and works in a cooperative manner with team.

• Participates and demonstrates an awareness of Entity and Departmental wide initiatives for Patient and Employee safety

when carrying out daily responsibilities.

• Validation of annual competencies required for the position.

• Performs selected procedures or assists with treatments and procedures as detailed on skills checklist and according to

PPMC’S policies and procedures.

• Maintains and utilizes unit supplies in cost-effective manner, as needed.

• Responds with flexibility to the needs of the unit.

• Collaborates with the registered nurse to plan, implement, and evaluate the plan of care.

• Documents observations and care rendered on the patient, and records information in a timely and accurate manner (flow

sheets, admission sheet).

• Handles patient’s valuables and personal belongings according to PPMC’s Policies and Procedures.

• Maintains knowledge of unit quality indicators and actively participates in quality goals.

• Performs daily glucose monitoring quality control.

• Safely transports and transfers the patient.

• Optimizes patient’s mobility.

• Maintains a clean and safe environment in patient care areas to include patient rooms, utility rooms, kitchen, hallways, and

nursing stations.

• Adheres to PPMC’s policies as it pertains to utilizing restraints and safety/falls policies

• Accurately documents patient’s vital signs, records I & O and reports and/or records observation rounds per PPMC’s policies

and procedures.

• Performs patient 1:1 observations and documents according to PPMC’s policies and procedures.

• Respects each patient's/customers personal dignity, right to privacy, and confidentiality.

• Demonstrates the knowledge and skill necessary to provide care/service based on physical, psychosocial, educational,

safety, and related criteria appropriate to the age of the patients/customers served in assigned area.

• Demonstrate an understanding of appropriate safety and infection control techniques and policy.

• Consistently exhibits an understanding of individual's role and how it relates to the delivery of care/service in the

organization.

• Performs administrative duties including but not limited to answering phones, responding to patient call bells, creating and

maintaining patient’s files, obtaining patient’s consents and ordering office supplies.

• Performs duties in accordance with Penn Medicine and entity values, policies, and procedures

• Other duties as assigned to support the unit, department, entity, and health system organization

Minimum Requirements:

H.S. Diploma/GED And PA State Certified Nursing Assistant Or Current nursing student who has successfully completed a clinical rotation in their ASN/BSN nursing program.

While matriculated in accredited nursing program, one must have minimally completed the nursing foundations/fundamentals clinical course with a 3.0 GPA overall

Valid BLS Certification through American Heart Association (AHA )

Essential Skills and Abilities:

• Adheres to professional image guidelines and has no more than a counseling notice in a 12-month period.

• Demonstrates respect for the patient's Bill of Rights.

• Maintains confidentiality in regard to patient care information and chart contents.

• Ensures and maintains patient safety.
