Virginia State Job Bank ACT Case Manager Petersburg, Virginia
District 19 Community Services Board (D19CSB) is seeking a qualified mental health professional to work as a generalist case manager for the interdisciplinary ACT (Assertive Community Treatment) team that serves adults with severe and persistent mental illness in the Petersburg/Tri Cities area. This program is designed to provide an Evidence-Based Practice model of highly coordinated set of services offered by a self-contained interdisciplinary community-based team of medical, behavioral health, and rehabilitation professionals.
Successful applicant will have the knowledge, skill, and ability to carry out rehabilitation and support functions, based on the Integrated Dual Disorder Treatment model, focusing on assessment and treatment, using principles of harm reduction, motivational interviewing, and CBT.
ACT case managers work as a clinical member of the ACT team to treat a variety of mental health symptoms/disorders, and duties include, but are not limited to:
-implementing treatment plans to support clinical necessity of ICT/ACT services -providing Intensive Community Treatment and care coordination -engaging in crisis intervention including facilitation of pre-screening for involuntary hospitalization as necessary -completing medication deliveries to severely and persistently mentally ill consumers who reside in the D19 catchment area
Position provides community-based psychiatric treatment and will require entering homes and/or community centers, medication delivery, medication education and monitoring activities, daily living assistance, injections, and family support and case management.
Successful applicant must be registered with the Board as a QMHP-A, or be registered with the respective boards as a QMHP-T or QPPMH, under supervision.
One year of experience working with dually diagnosed adult consumers in a mental health setting Have computer experience and experience with electronic records. .
Valid drivers license in the state of residence with a point balance of -6 or higher. Travel to consumers residences required, with agency vehicle provided or mileage reimbursement for personal vehicle.
Required to obtain and maintain CPR certification.
Must have the ability to tolerate light lifting, bending, stooping, reaching, and climbing stairs.
ACT is an essential program, operating 365 days of the year. Applicant must be able to work some evenings (until 6:00pm), weekends, and holidays, based on program needs, and work with a rotating on call schedule.
Salary:
Position is a Grade S-C8. Salary range $55,201.00 - $62,098.00.
$2,500 Sign on Bonus to qualified applicant. *Restrictions apply
Hours:
ACT is an essential program, operating 365 days of the year. Applicant must be able to work some evenings (until 6:00pm), weekends, and holidays, based on program needs, and work with a rotating on call schedule.
