Job Duties

Collects data from patient, family or significant others and records on Initial Data Base form.Contributes to the formulation and updating of plan of care and implements plan of care under the direction of the Registered Nurse.Gives direct nursing care to assigned patients (e.g., medications, treatments, dressing changes, TPR) according to established standards.Recognizes changes in patient condition and responds appropriately, notifying charge nurse as needed.Documents pertinent information regarding patient's responses and nursing interventions in patient's medical record.Administers medication according to hospital policiesPATIENT CARE RELATED DUTIESApplies principles of infection control and safety in the performance of patient care.Communicates effectively with charge nurse and other member of the Health Care Team regarding patient's needs and/or changes in condition.Responds appropriately in crisis situation.Reports significant unit problems to the R.N.Patient care responsibilities are delivered with a knowledge of patient growth and development and are appropriate to the ages of the patients served.Understands and adheres to the Medical Center’s Code of Conduct.Familiar with the Medical Center’s Mission, Vision, and Values StatementsMaintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental, and infection control standards. Promotes adherence to the Patient’s Bill of Rights.

Graduation from an accredited school of Practical Nursing or equivalent education as described by the State EducationLaw.Current NJ license as an LPN.Successful completion of Progressive Care Orientation Program required.Experience

None

SkillsBLS requiredClinical nursing skills appropriate for an LPN.Good oral and written communication skills.Good interpersonal skills.Speaks, reads and writes English to the extent required by the position.

JOB SETTING/PHYSICAL DEMANDSAll patient care areas; contact with patients, visitors, staff.Frequent prolonged standing/walking.Lifts, positions, pushes, and/or transfers patients.Protective clothing worn: face shield, gloves, impermeable gown, particulate respirator.
