Job Summary and Qualifications

The Wound Care Team Lead provides professional care in accordance with the multidisciplinary plan of care and in conjunction with physician treatment plans. This person conducts comprehensive assessment, establishes a plan of care for complex wound patients and conducts regular re-evaluation of prevention of skin care issues and of healing. Under general supervision, provides clinically proficient professional wound care and acts as a resource to other members of the health care team.

The Wound Care Team Lead has accountability for monitoring patient outcomes. As the coordinator of the patient wound care team (including but not limited to: physicians, registered nurses, licensed vocational nurses, technicians, Physical and Occupational Therapists, and support and clerical staff), the Wound Care Team Lead collaborates, directs, delegates, assigns, guides and serves as a resource to the department and the patient care team for patient care delivery.

The Wound Care Team Lead promotes a cooperative working relationship with care team members, physicians, other disciplines and the public by facilitating and enhancing communication, displaying honesty and respect, displaying sensitivity to cultural and age differences, and expressing and accepting feedback in a professional manner.

KNOWLEDGE, SKILLS, & ABILITIES:

  • Proficient – has ability to perceive and understand situations as whole parts; holistic understanding of patient care for improved decision-making; and ability to learn from experiences what to expect and how to modify plans (Benner-2001)

  • Ability to learn and utilize computerized patient care management system.

  • Ability to understand and follow verbal and written instructions. Must effectively read, write and verbally communicate in the English language.

  • Knowledge of desktop computers, infusion pumps, non-invasive vital sign monitoring equipment

  • Able to handle multiple priorities, independent decision making, critical thinker and works alone effectively as needed.

  • Manage stress appropriately.

  • Make decisions under pressure.

  • Manage anger/fear/hostility/violence of others appropriately.

  • Manage internal and external customers effectively.

  • Handle multiple priorities.

  • Work with a diverse mix of people.

What qualifications you will need:

  • Basic Cardiac Life Support must be obtained within 30 days of employment start date

  • Licensed Physical Therapist

  • Graduate of an accredited school of Physical or Occupational Therapy required. Masters degree required. Doctorate preferred.

EXPERIENCE:

  • One (1) year or more of full-time PT or OT experience; three (3) preferred.

  • Previous wound care experience.

CERTIFICATE/LICENSE:

  • Certified Wound Care Specialist or CWS or WCC

  • Current Nevada Physical or Occupational Therapy License. •

  • Current BCLS (American Heart Association or American Red Cross)

DUTIES INCLUDE BUT ARE NOT LIMITED TO:

  • The Wound Care Team Lead participates in the development, implementation and evaluation of the wound orientation program for new personnel and the clinical education of students and makes suggestions for specific changes to these programs.

  • The Wound Care Team Lead coordinates unit activities in support of wound care practices and promotes cost containment and appropriate utilization of staff, supplies and equipment resources and adheres to all policies and procedures.

  • The Wound Care Team Lead establishes goals for own professional development and seeks educational and patient care assignment opportunities necessary to meet developmental goals.

  • Assumes individual responsibility for cost effectiveness. Provides input into budgetary process.

  • Ensures a safe working environment. Complies with hospital life-safety, quality and risk management policies, standards and procedures, and in doing so is responsible for identifying, reporting and initiating corrective action.

  • The Wound Care Team Lead participates in the performance improvement processes, per The Joint Commission and/or other regulatory agencies.

OTHER JOB RESPONSIBILITIES:

  • Establishes protocol guidelines to correlate skin risk assessment and protocol

  • Establish protocol for management of skin issues (skin tears, incontinence related dermatitis, partial thickness and full thickness wounds including pressure injuries)

  • Evaluate and recommend skin and wound care products for various indications and set parameters for use

  • Conduct comprehensive assessment, establish plan of care for complex patients and conduct regular re-evaluations for healing.

  • Provide competency- based staff education for new employees and annual competency – based education for existing employees.

  • Track outcomes and identify skin and wound care issues, initiate quality improvement activities for wound care program and report annually.
