Job Summary:

Provide physical therapy services to individuals as outlined in their Individual Education Plan or Individual Family Service Plan. Services are to be provided in accordance with New York State practice act guidelines for Physical Therapy Services, the American Physical Therapy Association as well as in accordance with all company, state and federal standards and regulations; as well as within all regulations and requirements set forth by the organizations with which the company collaborates.

Essential Job Duties:

  1. Reviews all available documentation related to the individual prior to beginning treatment; including the IEP/IFSP, script and prior evaluations if available

  2. Develops an appropriate treatment plan for each individual

  3. Performs treatments according to IFSP/IEP and accepted professional standards and practices

  4. Conducts ongoing assessment of individual during each visit, and modifies goals/outcomes/treatment plan accordingly

  5. Performs appropriate parent/caregiver education and develops home programs as necessary

  6. Works as part of an inter-disciplinary team, utilizing communication with all team members to enhance the treatment program of the individual

  7. Maintains an accurate schedule

  8. Submits all required documentation of visits according to established procedures

  9. Performs evaluations as assigned and when necessary

  10. Communicates with families on a regular basis

  11. Keeps abreast of legal responsibilities, professional ethics, standards of practice and agency rules, policies and regulations.

  12. Provides for own professional growth by attending continuing education and in-services; tracking this to assure compliance with state licensing requirements

  13. Discontinues therapy when child no longer meets eligibility requirements

  14. Provides own transportation to service locations

  15. Maintains client confidentiality with HIPPA/FERPA and agency guidelines

  16. Maintains professional appearance, follows agency dress code

  17. Attends department meetings and any other meetings required by your supervisor

  18. Adheres to and signs Annual Employee Agreement

  19. Documents time off and time worked accurately according to agency policies

  20. Performs other duties and assumes other responsibilities as assigned by their direct supervisor

Skills:

• Basic computer skills

Education:

• Minimum Degree Required: Master’s Degree

Experience:

None required, but some is preferred.

License/Registration/ Certification:

• Current New York State Physical Therapy License and Registration

• Valid NYS Driver’s License

Training:

• Minimum of 36 hours of Continuing education per licensing period (3 years) as per NYS Office of Profession guidelines

• All required agency trainings

• All trainings required by programs you service individuals in, including but not limited to, OCFS and Early Intervention

Physical Requirements:

The frequent physical demands of the position are sitting, standing, walking, climbing, talking, listening, repetitive motions, lifting, carrying, pushing, pulling, balancing, stooping, crouching, reaching, handling of children and grasping.

Salary Range:

$55,614 - $74,149/annually based on applicable experience
