PeaceHealth Program Coordinator Diabetes Education in Ketchikan, Alaska

Description

PeaceHealth is seeking a Program Coordinator Diabetes Education for a Part Time, 0.80 FTE, Day position. The salary range for this job opening at PeaceHealth is $47.85 – $71.77. The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc.

DETAILS OF THE POSITION:

  • Oversees the planning, implementation and evaluation of the diabetes education program in partnership with the department leadership. Designs the intent, scope, action plan, and objectives for effective learning.

  • Reviews policies and procedures related to diabetes care, state and federal laws/regulations and diabetes standards of care. Assures national diabetes education recognition/certification standards are met. Responsible for maintaining recognition/certification and participates in the evaluation of program effectiveness.

  • Collaborates with physicians/providers, managers and staff by providing consulation and leadership, promoting coordination and standardization of patient/family information. Facilitates coordination of patient care by conducting interdisciplinary care conferences as needed/indicated.

  • Ensures effective delivery and coordination of quality patient care by clinical oversight through conferences, medical record review, and patient/family interviews.

  • Performs initial and follow-up nutrition assessments, develops care plans, makes referrals, provides family teaching, counseling, and completes documentation on patients.

  • Facilitates or serves on committees/task forces within Peacehealth and the community to promotediabetes education, clinical coordination, and CQI.

  • Develops and facilitates the design and development of diabetes curriculum to educate staff on learning needs of patients (cultural differences, specific age group needs, identifying learning barriers) Supports training of instructors and educators throughout Peacehealth. Provides education appropriate to patient’s needs.

  • Performs other duties as assigned.

WHAT YOU BRING:

  • Bachelor's Degree Preferred: in related field

  • 1 year Required: Diabetes education (or equivalent education and/or experience) and

  • Required: Previous experience in program management, development of training and educational material, and quality improvement initiatives and

  • Required: Academic and/or experience in the healthcare of persons with chronic disease and

  • Preferred: Experience in outpatient clinical practice

  • Required Upon Hire: Certified Diabetes Care and Education Specialist (CDCES) and

  • Required Upon Hire: Basic Life Support

  • Demonstrated ability to promote positive interpersonal relationships and patient/family centered work environment. (Required)

  • Basic computer skills including MS Office applications. Experience with relevant software programs. (Required)

  • Analytical and problem-solving skills. (Required)

  • Good communication skills, both verbal and written. (Required)

  • Familiarity medical terminology, and knowledge of diabetes assessment/care. (Required)

  • Ability to teach/train and develop educational programs for a wide range of individuals and learning styles. (Required)
