Wellspring Lutheran Services Case Manager in Kentwood, Michigan

Hello! Thank you for your interest in joining the Wellspring team as a Case Manager with the Foster Care Program. As a Foster Care Case Manager with Wellspring you'll be part of a team of social workers committed to helping families and children in Kent, Kalamazoo and Muskegon Counties. In this position, you'll be directly supporting communities across the region to help families and children get the resources they need. You will have a bachelor's degree in social work or a human services related field, a current Michigan driver's license, car registration and auto insurance. You will also be reliable, and have a responsible attitude, a compassionate approach and a commitment to quality in everything you do. 

As a Case Manager, you will:

  • Coordinate the development, review and implementation of services, case management, safety/behavior intervention plans and treatments provided to individuals served.
  • Coordinate all community referrals, resources and services to the family.
  • Oversee all services provided to individuals and their families, monitor the quality of care, ensure compliance with contractual terms and monitor regulatory program compliance.
  • Coordinate and monitor the implementation of required medical and dental services for all foster children.
  • Visit each assigned child at least once each month in the foster home.
  • Maintain accurate records, confidentiality and timely reports.
  • Prepare court reports and petitions, and attend court hearings to provide testimony to support the permanency, safety and well-being of the child/family.
  • Coordinate visitation and supervise children with parents, siblings and relatives.
  • Act as a liaison with schools and other community agencies.
  • Attend staff meetings and treatment planning meetings as required.
  • Administer drug screens.
  • Transport clients (as needed).
  • Engage and encourage family member participation in services and case planning to address needs.
  • Provide emergency, on-call interventions after hours and on holidays, as required.
  • Integrate learning from required DHHS training into day-to-day responsibilities.
