CommuniCare is currently looking to fill a key position, Director of Veteran Programs and Services.

Estimated Salary Range: $95k-115k/annually

PURPOSE/BELIEF STATEMENT

The position of Director of Veteran Programs and Services leads CommuniCare’s initiative of serving those who served. This position is directly involved in program development, implementation, oversight, education, and expansion. This position supports programs which provide customized clinical care, emotional support, and meaningful military connections that relay gratitude and promote dignity to residents.

The Director of Veteran Program and Services maintains strong relationships with VA Hospitals, offices, and providers. This position provides education and support to Veteran affiliated community providers. They develop solutions that meet identified provider needs. The efforts of this position work in tandem with clinical, operational, and business development leadership.

JOB DUTIES & RESPONSIBILITIES

  • Lead Veteran Programs and Services

  • Oversee specialized veteran’s care and services in a post-acute care setting.

  • Work with interdisciplinary teams on program implementation.

  • Responsible for building program capacity through outreach with programmatic partners.

  • Monitor and evaluate the effectiveness of program initiatives.

  • Responsible to identify gaps in any program or service model and work with interdisciplinary teams to develop enhancements.

  • Coordinate Veteran Recognition Events.

  • Oversee coordination of events and activities for veterans and/or veterans and their families.

  • Maintain a master annual schedule of events.

  • Engage in Community Outreach.

  • Provide presentations about veteran programs to veteran service organizations (VSOs), community groups, fraternal organizations, emergency responders, military installations, and all other identified groups.

  • Facilitate vet to vet relationships.

  • Engage in Business Development.

  • Assist in developing marketing materials and communication strategies.

  • Establish and maintain relationships with VA Hospitals, providers, and affiliate offices.

  • Establish and maintain relationships with VA contract personnel.

  • Develop and maintain quarterly business development plans and market analysis.

  • Perform other related activities as assigned or requested.

QUALIFICATIONS & EXPERIENCE REQUIREMENTS

  • Strong familiarity with veteran’s resources, benefits, and organizations.

  • Experience working with the military and veteran population.

  • 5+ years in clinical social work, nursing, or veteran healthcare related field preferred.

  • Business development experience preferred.

KNOWLEDGE/SKILLS/ABILITIES

  • Must understand the complex and ongoing issues facing those who have served.

  • Knowledge of post-acute and long-term care as well as veteran support services.

  • Ability to work independently in a fast-paced environment and multi-task.

  • Must possess a desire to learn and execute the sales process in marketing or health care related field.

  • Critical care, rehabilitation, third party reimbursement, and/or case management experience.

  • Must have the ability to make independent decisions when circumstances warrant such action.

  • Must be able to deal tactfully with team members, other staff, residents, family members, visitors, government agencies, and the general public

  • Excellent written and oral communication, interpersonal relationship skills, and computer literacy.
