Queen's Health System Sr. Discharge Coordinator - Social Work (Full-Time, Day, 40) in Honolulu, Hawaii

RESPONSIBILITIES

I. JOB SUMMARY/RESPONSIBILITIES:

• Plans, coordinates, and facilitates the daily activities of the Discharge Coordinators and ensures all activities are performed in accordance with established policies and procedures.

• Provides work leadership and guidance to the Discharge Coordinators.

• Ensures quality service and patient care in the department.

• Perform duties of a Discharge Coordinator.

II. TYPICAL PHYSICAL DEMANDS:

• Essential: sitting, finger dexterity, seeing, hearing, speaking, frequent gripping of an object.

• Continuous: static gripping of an object for prolonged periods.

• Frequent: walking, lifting usual weight of 3 pounds; carrying usual weight of 2 pounds; reaching at shoulder level; repetitive arm/hand motions.

• Occasional: standing, stooping/bending, kneeling, climbing stairs, walking on uneven ground, squatting, twisting body, lifting maximum of 10 pounds, reaching above and below shoulder level.

• Operates calculator, computer, printer, telephone, and facsimile.

III. TYPICAL WORKING CONDITIONS:

• Not substantially subjected to adverse environmental conditions.

• Subjected to noise and/or vibration due to shared office space, air conditioning and construction work in neighboring department.

• May be exposed to hostile, disruptive, assaultive and inappropriate patient behavior.

IV. MINIMUM QUALIFICATIONS:

A. EDUCATION/CERTIFICATION AND LICENSURE:

• Bachelor’s degree in Social Work or related field; or a bachelor’s degree in a non-related field with two (2) years of experience working in a comparable job demonstrating knowledge of governmental and social agencies and services may be substituted for degree in Social Work or related field.

B. EXPERIENCE:

• In addition to the education requirement, three (3) years experience as a Discharge Coordinator or in Social Services.

• Prior supervisory/leadership experience preferred.
