As a Physical Therapist, you will have the responsibility of providing exceptional and personalized care to our patients in the comfortable setting of their own homes. This role offers the flexibility to create your own schedule, allowing you to align it with your personal needs and preferences. Responsibilities: * Conduct comprehensive assessments to determine the unique needs of each patient. * Develop and modify personalized treatment plans in collaboration with physicians, based on patient progress. * Contribute to the development and implementation of interdisciplinary patient care plans specifically tailored to physical therapy. * Administer diagnostic and prognostic tests to evaluate the muscle, nerve, joint, and functional abilities of patients. * Effectively alleviate pain, enhance function, and optimize performance through a variety of advanced physical therapy techniques. * Educate and guide patients and their families in the physical therapy program. * Coordinate outpatient services for procedures that cannot be conducted at home. * Maintain accurate and timely records of evaluations, treatments, and patients' therapy intervention responses. * Provide supervision to Physical Therapy Assistants and paraprofessionals. Requirements: * Graduate of a Physical Therapy school approved by the Council of Medical Education of the American Medical Association, in collaboration with the American Physical Therapy Association. * Hold a valid license to practice Physical Therapy in the state of Hawaii. * Minimum of one year of institutional/home health experience (preferred). * Successfully complete a comprehensive criminal background check. * Possess a current CPR certification.
