Job Overview:

Responsible for the development, coordination, promotion, execution and presentation of Preventive Health Programs for Community members with utilization of evidenced based clinical information. Responsible for coordinating and completing various health screenings and programs for community members with timely and appropriate follow up. Works collaboratively with MAP physicians, midwives, nutritionists and other health educators to develop and execute educational programs for the community.

Qualifications:

Required:

  • Graduate of accredited School of Nursing

  • Minimum of 3 years Nursing and Education experience

Preferred:

  • Bilingual

  • Bachelor's degree

Certifications and Licenses Required:

  • CPR certificate

  • Current RN license in the state of NJ

Scheduling Requirements:

  • Shift working, Varies

  • Monday - Friday

  • Full Time

Essential Functions:

  • Demonstrates an understanding of needs of the community members and presents health programs focusing in on a variety of health issues by using clinically based information and networking with healthcare providers and outside organizations

  • Perform ongoing evaluations of the programs, the instructors and participants to ensure the program meets the needs of the community with successful program satisfaction scores that consistently reach the departmental benchmark of 95 percent

  • Works collaboratively with other health educators and healthcare providers to incorporate updated preventive health issues in programs

  • Actively takes ownership in the development, implementation and presentation of these programs and becomes known as the expert in the community of Preventive Health topics

  • Actively participates in 7 major Community Health Special Events

  • Maintains a flexible schedule with the ability to work evenings and weekends as needed

  • Responsible to ensuring that there are a limited number of canceled programs per year due to low attendance

  • Actively demonstrates the leadership role in educating family members in detail by reviewing health information and explaining results to screening participants

  • Consistently attends Monday Morning Huddle and Staff Meetings, and contributes as a team member

  • Works with PR Marketing when appropriate and creatively self-promotes programs to assist in increasing attendance

  • Champions opportunities for self-directed continuing educational opportunities

  • Work collaboratively with the Care Transition Team to follow up with phone calls and home visits to patients identified by The CTT For Education on CDSM

  • Coordinates and conducts 8 cancer and 8 non cancer health screenings. Part Time - 4/4. Assures that appropriate follow up is maintained according to the departmental policies
