4-year college degree Minimum 2 years working in jail system strongly preferred; court system or other criminal justice arena considered Demonstrated commitment to care for vulnerable patients Demonstrated career Interest in issues in women's issues, LGBTQI+ issues, health advocacy, compassionate release policy, and/or incarceration One or more years' experience managing direct coordination of care providers Facility with excel, electronic medical records and other relevant software 1. Four years of full-time experience in counseling, community work or community health activities in a government agency or community organization engaged in providing community services to the public, assisting members of the community in obtaining community services or maintaining liaison with schools, community organizations or other government agencies for the purpose of providing assistance and obtaining participation and support for implementation of community or public service programs; or, Complete intake assessments and transition plans for patients assigned to case load (includes patients in all populations served by TWIGS). Provide production list to facility HSA; meet with patients in clinic to complete interview and obtain consents. When necessary, meet with patients in housing area (DOC escorts not provided; staff may travel in pairs/groups; seek support and supervision around patient prioritization and safety) Complete document in electronic medical records system (CHER) within 3 business days of encounter; scan and upload relevant consents and documents. Escalate any urgent issues to supervisor. Provide ongoing counseling to patients on caseload. Develop plan with patient; discuss in team rounds and supervision when needed. Provide ongoing in-jail support to patients, meeting with them regularly; frequency to be decided in consultation with Program Manager. Prepare patients/cases to discuss Troubleshoot issues (e.g., pharmacy issues, medical appointments, referrals) as able; confer with supervisor as needed.
