UPMC Home Care Aide in Coudersport, Pennsylvania

UPMC Home Health is hiring a full-time Home Care Aide for the Coudersport Area.

  • Communicates appropriately with the interdisciplinary home care team promptly, reporting any changes in the patient's condition to the appropriate professional staff.

  • Attends compliance training and adheres to the organization's standards of conduct, policies, and procedures.

  • Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction.

  • Provides safe and direct personal care to patients in the home setting under the direct supervision of appropriate professionals.

  • Performs duties and job responsibilities in a fashion that coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians, and other departments within the organization.

  • Identifies, develops, and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide.

  • Proven ability to facilitate change.

  • Maintains and completes documentation accurately and promptly while in the patient's home and within the agency and regulatory standards.

  • Participates in all required agency meetings.

  • Must maintain annual competency evaluation scores as determined by regulatory requirements

High School Diploma or GED - Required

One (1) year experience - Preferred

Direct Patient Care required

Certified Nurse Aide -Preferred

A minimum of 75 hours of Nurse Aide training is required. If no previous nurse aide training, the applicant may attend the UPMC Home Healthcare Nurse Aide Training Course as available upon hire.

Licensure, Certifications, and Clearances:

Preferred: Nurse Aide Registry in good standing either in the state where the facility is located or in a state covered by a licensure compact agreement with the state where the facility is located. Required: CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire

  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
