Texas Health Resources Care Transition Supervisor - FT Days in Cleburne, Texas

Work location: Texas Health Cleburne 201 Walls Dr. Cleburne, TX 76033

Work hours: Full Time Days, 40 hours, Monday- Friday 8:00am-4:30pm, occasional weekend day coverage, which would allow a flex day during the week

Department highlights:

· Robust Unit Based Council (UBC) and engaged leadership team

  • Provides leadership and oversight to Care Transition Team(nurses and social workers) that provide care and resources to patients across various units such as ED, ICU, IMC and Med/surg .

Here’s What You Need

· BSN or Masters Degree In Social Work required

· TX RN upon Hire OR

· LMSW – Licensed Master Social Worker upon hire required

· CPR or BLS upon hire required

· ACM – Accredited Case Manager upon hire OR

CCM- Certified Case Manager upon hire OR other ANCC upon hire preferred

· 3+ years registered nurse experience in acute care hospital or 3+ years hospital/medical social work (if Social Worker) Required

· 2+ years hospital discharge planning/care management required

· Case management supervisory background preferred.

· Leadership experience preferred.

Skills

· Working knowledge of medical necessity criteria preferred

· Knowledge of MS Outlook and Office (Word, Excel)

· Customer Service Skills

· Ability to engage in complex clinical decision -making

· Strong oral and written communication skills

· Strong commitment to interdisciplinary collaboration

· Critical thinking, analysis and conflict resolution skills

· Psychosocial and crisis intervention skills

· Ability to prioritize and meet deadlines

Position Responsibilities

*Have basic oversight of departmental activities as well as monitoring of work performance of department

*Assures staff documentation is completed per guidelines as evidenced by appropriate monitoring techniques and staff coached as needed

*Assists leadership to implement practice changes successfully

*Reports pertinent quality/risk issues to appropriate individuals, departments and/or committees in a timely manner

*Participates on performance improvement teams as appropriate

*Collaborates with physician advisor as needed

*Participates in hospital/medical staff meetings to review patients meeting criteria related to high dollar/extended stay or difficult placement.

*Assists leadership to balance schedules according to departmental needs

*Resolves daily staffing issues, seeking assistance from leadership as needed

  • Be responsible for ensuring patients are transitioned to appropriate levels of care in a timely and effective manner

  • Ability to prioritize and meet deadlines

  • Communicate with the multidisciplinary team (physicians, nursing, therapy), patient, family and post-acute care stakeholders in order to coordinate care.

  • Observes, mentors and communicates concerns about staff performance

  • Serve as a point of contact for all identified stakeholders.

  • Execute and update the discharge plan as needed.

  • Facilitates care conferences for complex transitions, placement and palliative care needs.

  • Ensures patients are provided post-acute options based on clinical necessity and patient choice while also considering the payor source

  • Has a working knowledge of the following documents: Advanced Directives, Medical Power of Attorney, Application for Temporary Mental Health Treatment, and out-of-hospital Do Not Resuscitate.

  • Compliance requirements for delivery of HINN, second IMM and MOON letters

  • Potential denials, avoidable days, and alternate level of care days

  • Assists leadership to remedy actual vs. expected customer satisfaction performance gaps as needed
