Integrative Emergency Services Community Health Coordinator Carrollton, Texas
POSITION SUMMARY
Key Medical Care is seeking a Community Health Coordinator whose role will be dedicated to guiding elderly patients and their families in increasing their capacity to access healthcare services that address their unique needs. The Community Health Coordinator will assist patients in navigating the complexities of healthcare which involves coordinating care, facilitating communication among healthcare providers, and ensuring that geriatric patients receive personalized, compassionate support throughout their healthcare journey. This position will play a critical role in enhancing the quality of care, improving patient outcomes, and fostering a positive patient experience
The KMC team consists of board-certified physicians, nurse practitioners, nurses, clinical assistants, and social workers, all skilled in addressing immediate acute-care and chronic healthcare needs. ESSENTIAL DUTIES AND RESPONSIBILITIES

Patient Assessment: Ability to connect with patients and learn about their health history, needs, and goals. Practice active and empathic listening and communicate in a respectful manner and with cultural competency
Identifying Social Determinations of Health (SDOH): Ability to identify the non-medical factors that impact a patient’s health and longevity. Factors can be related, but not limited to income, education, housing, food, transportation, cultural values, and social support.
Education and Advocacy: Educate patients and families about available programs and services within our continuum of care .
Care Coordination: Collaborate with interdisciplinary teams, including clinical, social, and community workers, to coordinate care across multiple settings and specialties .
Data Collection: Ability to document and organize HIPAA compliant reports to help identify health and social trends and further refine networks and resources .
Appointment Management: Assist patients in scheduling appointments, coordinating transportation, and managing follow-up care to ensure timely access to services .
Health Promotion: Promote preventive care and healthy aging by providing information about wellness programs, screenings, and community resources .
Crisis Intervention: I dentify and address barriers to care, such as financial issues, transportation challenges, or caregiver support needs. Provide crisis intervention when necessary
Documentation: Maintain accurate and confidential records of patient interactions, care plans, and referrals in compliance with organizational policies and regulations
QUALIFICATIONS

Knowledgeable of and maintains HIPAA standards of privacy and confidentiality. Ability to use discretion appropriately and maintain confidentiality
Education / Experience: Include minimum education, technical training, and/or experience preferred to perform the job.
Required:
High School Diploma or GED equivalent
Minimum 1 year of experience in a customer service or patient focused environment
Valid Texas Driver’s license
Must have reliable transportation
Preferred:
2+ years experience with clinical, case management, or related duties
Degree in Psychology, Sociology, Community Health, or related field of study
2+ years’ experience in experience in health education, patient navigation, and/or health clinic patient assistance field
Community Health Worker (CHW): CHW license by the State of Texas
Bilingual (Vietnamese, Chinese, Arabic and/or Spanish) with English

While performing the duties of this job, the employee is regularly required to stand, sit, talk, hear, stoop, kneel, and use hands and fingers to operate a computer and telephone keyboard reach.
Frequently required to stand, walk, sit, use hands to feel, and reach with hands and arms . Includes ability to walk through hospital-based departments across broad campus settings, including Emergency Department environments
Possess the ability to fulfill any office activities normally expected in an office setting, to include, but not limited to: remaining seated for periods of time to perform computer based work, participating in filing activity, lifting and carrying office supplies (paper reams, mail, etc.)
Light to moderate lifting 20-25 pounds
Fine hand manipulation (keyboarding)

Key Medical is a smoke free workplace in compliance with federal guidelines.
Position will need to adhere to any immunization, vaccination, or other requirements in place at our client sites in accordance with their policies and procedures.
EXPECTED PAY:
18 - 22 USD per hour depending on experience and qualifications
EXPECTED HOURS:
Shifts will typically range from 8-10 hours
Shifts will typically be either a 1st shift or 2nd shift
Flexibility to work rotating weekends is desired
TRAVEL:
Primary worksite at Carrollton Regional Medical Center - 4343 N Josey Ln, Carrollton, TX 75010
Initial training (1 week) and occasional travel to Silverstone Health Office in downtown Dallas - 4100 Harry Hines Blvd Suite #375, Dallas, TX 75219
