Berkshire Farm Center Assistant Bilingual Lead Case Manager in Canaan, New York

Responsible for coordinating case management and safe and timely release services, training new case managers, and supporting the Lead Case Manager with supervising the work of other case managers.

Job Responsibilities

• Responsible for day-to-day supervision of case managers including coordinating and/or conducting staff training and development, directing and assigning work when LCM is not working, managing work environments, , and making recommendations for other personnel actions to LCM.

• Ensures and monitors delivery of consistent and comprehensive sets of services are provided to youth and families at frequencies consistent with treatment goals, state mandates, and Agency policy.

• Ensures all required documentation is completed in a timely, consistent and comprehensive manner.

• Responsible to receive and report all Critical Incidents timely. Manage and reduce risk in area(s) of responsibility and to report it if it is outside of area(s) of responsibility or seek assistance when needed to resolve the matter.

• Assures health and safety risks are controlled or removed, and a safe and therapeutic environment exists.

• Responsible for developing process and program improvement plans by utilizing data driven output and/or information. Systematically monitors results against expectations and implements/recommends a corrective action plan where necessary including developing systems for tracking reports and other matters.

• Recommends and implements new programming or processes as identified.

• Assist with developing annual budget; monitors and develops administration of program budget. Monitors programs and services meet/exceed utilization targets and expenses do not exceed budgetary limits. Participates in development of budget based on current spending and trends.

• Serves as the face of Berkshire while promoting the system of care for youth needing higher or lower services. Participates in and initiates program development marketing efforts. Monitors environmental trends in order to meet and expand programming needs.

• Oversees completion of federally mandated and agency documentation according to policy and regulatory requirements, in a fast-paced/time-sensitive environment.

• Maintains positive partnership with federal liaison, including coordinating required services for youth and families and overseeing timely and effective communication regarding case activities on a daily/weekly basis.

• Oversees provision of crisis intervention services for youth experiencing acute distress and dysfunctional behaviors to address safety concerns and documentation of SIRs.

• Oversees completion of supportive case work support to sponsors, including assessments, and detail-oriented travel planning for youth.

• Complies will all federal, state, and local laws and regulations, including those or ORR and the NYS Office of Children and Family Services

• Adheres to agency policies and procedures; follows and supports agency mission, vision and values including participating in internal surveys and data collection for continuous improvement initiatives.

Job Requirements

• Master’s degree in the behavioral sciences, human services or social services preferred. Bachelor’s degree required and case management experience.

• Fluent in English/Spanish with excellent verbal and written communication skills

• Must maintain First Aid/CPR and TCI certification.

• Must have a current, valid driver’s license per Agency guidelines. Travel required

• Must be able to work a flexible schedule, outside of normal business hours including nights, weekends, and on-call, as needed.

• Ability to work with economically and culturally diverse population.

Schedule

Sunday through Thursday 9AM- 5PM.

Pay

$ 63,240 annually
