Description
Comunilife Incis looking to fill a Case Manager position for its Bronx residential program. The Case Manager develops supportive relationships with clients; link them to community- based medical and mental health care, and provide crisis intervention as needed. The Case Manager provides support with accessing primary medical care and other necessary services to increase clients’ capacity for independent living and monitors progress, providing referrals as needed. The Case Manager completes required documentation in a timely manner and participates in case conferences, on-site and community-based meetings and trainings as required. Essential duties:
Completing comprehensive assessments, service plans, progress notes
Monitoring and reassess clients’ needs for referrals and services
Provide psychoeducation on relevant topics like substance use disorders, entitlements, etc.
Provides individual and group workshops
Work collaboratively in an inter-disciplinary team to ensure comprehensive, client-centered services are delivered and documented.
Qualifications
Education and Experience Requirement(s):
Bachelor's Degree in human services or related field; Credentialed Alcohol and Substance
Abuse Counseling (CASAC-T or CASAC) is a plus
Two (2) years' work experience
Experience with HIV/AIDS afflicted individuals, chronic mental illness, substance abuse individuals and their families a plus
Bilingual skills in English/Spanish a plus
Knowledge and hands-on experience in providing direct services related to the treatment
and recovery of persons with HIV/AIDS, substance abuse, serious mental illness, and homelessness
Excellent oral and written communication skills; proficiency with Ms Office and Google
product suites
Ability to perform and manage in a team approach; excellent interpersonal skills
Ability to handle complex challenges and balance multiple demands on time and attention
Ability and willingness to perform all duties as workload necessitates
