UR/Discharge Planning/Swing Bed Coordinator/Employee Health and Infection Prevention

The UR/Discharge Planning/Swing bed Coord/EE Health and Infection Prevention RN collaborates as part of the health care team with the Case Management Social Worker to assess the patients plan of care and expected outcomes with special attention being given towards the progression of discharge services. Analyzes patient records to determine legitimacy of admission, treatment, and length of stay to comply with government and insurance companies reimbursement policies. Oversees the admission process including the initial nursing assessment and plan of care. Works collaboratively with the Providers and clinical staff to identify current standards of practice, educations and research to meet evidenced based practice and regulatory requirements. Takes action to improve the effectiveness of clinical practice and outcomes of care. As Employee Health Nurse provides direct and indirect nursing care for Melham employees and contractors. As Infection Preventionist; leads and facilitates the provision of excellent, safe evidence-based, efficient infection prevention and control practices and standards to achieve optimum patient outcomes; facilitates compliance with regulatory and accreditation organizations; identification of infectious disease processes; surveillance and epidemiologic investigation; preventing and controlling the transmission of infectious agents; works effectively and collaboratively with other patient care teams and providers.

This is a full time, 40 hours per week position. A sign on bonus of $7,000 is offered with this position.

Qualifications/Requirements

Graduate of School of Nursing; BSN or BA required

Approximately three-five years progressively more responsible clinical work-related experience

Current NE State License as a Registered Nurse required or ability to obtain

Professional knowledge of nursing practice, and regulations to support patient care delivery, evidence-based practice, utilization review and case management

Interpersonal skills necessary to provide effective leadership to subordinates and effectively communicate with a wide variety of hospital personnel, community residents, and outside agency personnel

Analytical skills necessary in order to develop and prepare special and recurring reports or analysis

Current BLS, ACLS, PALS or ability to obtain within 2 months of hire

Current TNCC or ability to obtain within 9 months of hire

Job functions must be performed on site with regular attendance

Requirements for UR/Discharge Planner/Swing bed Coordinator

The UR/Discharge Planner/Swing bed Coordinator functions to create an environment of patient focused care, defining the model of case management, including compliance with CMS Condition of Participation regarding Utilization Review and Discharge Planning.

Significant Duties

Oversees the daily operations of Case Management Services (Case Management RN, Case Management SW, SWING Intake Liaison, and Clinical Guideline Management).

Collaborates as part of the health care team with the Case Management Social Worker to assess the patients plan of care and expected outcomes with special attention being given towards the progression of discharge services.

Is responsible for the referral intake process of potential SWING bed patients including collaboration for the acceptance of patients with the Interdisciplinary team.

Analyzes patient records to determine legitimacy of admission, treatment, and length of stay to comply with government and insurance companies reimbursement policies.

Oversees the admission process including the initial nursing assessment and Plan of Care.

Works collaboratively with the Providers and clinical staff to identify current standards of practice, educations and research to meet evidenced based practice and regulatory requirements.

Takes action to improve the effectiveness of clinical ractice and outcomes of care.

Direct discharge planning process (home care needs) with health care team in support of treatment adherence and medication compliance; assist with transitions for patients discharged from hospital setting to alternate home care environment

Requirements for Employee Health/Infection Preventionist

The Employee Health Nurse provides direct and indirect nursing care for JMMMC employees and contractors. Utilizes established protocols and current standards of care to meet the wide variety of health care needs of the JMMMC workforce population.

As Infection Preventionist; leads and facilitates the provision of excellent, safe evidence-based, efficient infection prevention and control practices and standards to achieve optimum patient outcomes; facilitates compliance with regulatory and accreditation organizations; identification of infectious disease processes; surveillance and epidemiologic investigation; preventing and controlling the transmission of infectious agents; works effectively and collaboratively with other patient care teams and providers.

Significant Duties for Employee Health

Monitors employee work-related illness and injury trends and assesses, develops and implements strategies to prevent work-acquired injury or illness and expedite return to work.

Performs bloodborne pathogen exposure assessment, treatment and follow-up.

Participates in preventative screening programs (e.g. TB skin testing, Influenza, Hepatitis B, MMR, Varicella and other appropriate immunizations) and surveillance programs designed to identify and monitor risks to the employees health.

Conducts workplace visits to identify potential hazards harmful to the workers health with regard to the work environment, processes and equipment handling.

Develops strategies for programs to increase employee's awareness of workplace hazards, the effect the hazards on their health and protective equipment to be used to protect the employee from hazards.

Safely administers and appropriately documents on immunizations and TB skin tests given to employees per protocol.

Documents employee health information thoroughly and accurately; maintains and ensures confidentiality of employee health information and medical records.

Acts as an advocate for the rights of employees by ensuring access to preventative health and safety measures.

Facilitates and implements Employee Health Program, based on current trends to ensure compliance with OSHA, Department of Labor, CDC, CMS and other requirements.

Participates in Safety Committee, Infection Prevention and Control Committee and other committees, task forces, or improvement projects, as appropriate.

Updates and maintains Employee Health policies and procedures.

Significant Duties for Infection Preventionist

Conducts on-going surveillance and documentation of infections, utilizing priority-directed, targeted surveillance method to ensure safe outcomes.

Responsible for mandatory infectious disease data reporting and surveillance and data collection/analysis of infectious disease data.

Notify local, County and/or State Health Department of any reportables diseases, according to regulations.

Assists in the investigation of clusters of infection when identified to decrease further transmission of communicable diseases.

Conducts risk assessment including Infection Prevention and Control program, TB and other specific, ad hoc risk assessments.

Adopts/develops and implements policies, procedures, work practices which prevent and control the transmission of infectious agents.
