POSITION PORPOSE

  • The Nurse Navigator / Multidisciplinary Care Coordinator position for a Registered Nurse to be a and patient/client advocate, who assists patients admitted with stroke and other neurological diseases.

  • The Neuroscience Nurse Navigator is a key member of the multidisciplinary team functioning as an advocate, educator, and facilitator of high-quality care.

  • They are responsible for assisting with referrals to comprehensive services and helping patients and their families to overcome barriers by facilitating timely access to care and providing education and support.

  • Full-time, benefited position. Scheduled 40 hours per week on the Day shift. No Weekends or Holidays. Ability to work remote one day per week.

WHAT YOU WILL DO

  • Assists with selected clinical and administrative aspects, initiatives and responsibilities associated with the neuroscience program.

  • Reviews care delivered, and collects data where appropriate, surrounding the alignment of key priority metrics, such as patients that have received thrombolytics or post-thrombectomy, and develops/ maintains monitoring systems to improve patient outcomes.

  • Assists in team training, colleague education and rounding activities on neuroscience units, in clinical or care conferences and participates in discussions around current and post-discharge care.

  • Understands and helps the interdisciplinary team to develops goals, and treatment plans in accordance with clinical practice guidelines and standards of care.

  • Uses critical thinking and problem-solving skills to analyze and synthesize data and assist in the formation of plans of care.

  • Coordinates and/ or assists with timely diagnostics for the stroke and neuro-intervention program.

  • Navigates/monitors all neurointerventional patients undergoing multimodality therapy.

  • Responds to code stroke alerts.

  • Understands regulatory and accreditation standards (e.g. Joint Commission, CMS) for stroke centers and assists in meeting these standards consistently.

  • Member of the Acute Stroke Team

MINIUMUM QUALIFICATIONS

  • Current Michigan Nursing License

  • Graduation from accredited nursing program: BSN Required.

  • Three or more years of progressive clinical nursing experience in Neurosciences or related field and/or experience.

  • NIHSS Certification obtained within 3 months of hire and maintained annually.

  • CNRN or CSN certifications are strongly preferred.
