Summary

This position provides oversight of Maternal Infant Health Program (MIHP) RNs, social workers, dietician, MIHP Referral Coordinator, certified pediatric nurses/MVN Mom-Baby program nurses, and others as determined by the Clinical Nursing Director. Responsibilities include facilitating the delivery of effective, cost-efficient, high quality, professional home care services; implementing department policies and procedures in accordance with regulatory, accreditation and practice standards; and clinical management of all relevant OB and pediatric clinical programs in collaboration with the Clinical Nursing Director and others.

CHARACTERISTIC DUTIES

  • Develops, evaluates, and modifies a unit staffing plan to meet patient care needs.

  • Provides direct supervision and evaluation of staff including registered nurses, social work, referral coordinator, dietician.

  • Understands and implements bargaining agreements for UMPNC employees, and manages other staff through the Standard Practice Guide, as well as PAC MVN policies. Ensures that consistent practices are followed.

  • Provides oversight of all patient care services and personnel, including making patient and personnel assignments, coordinating patient care, and coordinating referrals.

  • Assures that patient needs are continually assessed and oversees the coordination of patient care. Knowledgeable about patient caseloads and practice issues of staff.

  • Manages team performance based on established metrics and using data in decision-making.

  • Holds staff accountable to work and productivity expectations.

  • Performs skilled intermittent visits when necessary to meet the needs of patients.

  • Works collaboratively with other Home Care Services staff, care management, intake, quality, billing and related departments to coordinate care, build relationships and facilitate effective discharge planning and provision of patient care.

  • Provides weekend, holiday, and evening supervisory coverage as assigned.

  • Assists in developing, implementing, and participating in staff education programs, policies, guidelines, orientation and in-services.

Responsibilities*

Essential

  • Managers the MIHP, Mom-Baby and Certified Pediatric Programs at Michigan Visiting Nurses.

  • Understands and implements bargaining agreements for UMPNC and AFSCME employees, and manages other staff through the Standard Practice Guide, as well as PACS and MVN policy. Ensures that consistent practices are followed.

  • Assures MIHP program is in compliance with Medicaid requirements, and applicable federal, state, and Michigan Medicine regulations, policies, procedures and guidelines.

  • Provides direct supervision and evaluation of staff including nurses, social workers, dietitians, and other classifications as assigned.

  • Supervises staff and evaluates job performance on a regular basis through objective review. Writes, updates, and enforces internal policies, procedures, guidelines, and protocols that comply with requirements.

  • Coordinates the program, develops and/or monitors contracts, produces reports, manages crisis situations.

  • Manages program(s) within an approved budget and always maintains fiscal responsibility.

  • Demonstrates desired results in achievement of all relevant strategic, budgetary, and operational targets and objectives including, but not limited to, revenue, expense, volume, utilization, engagement/satisfaction, and quality indicators.

  • Provides weekend, holiday, and evening supervisory on-call coverage within MVN as assigned.

  • Develops the program budget, staffing requirements and ensures the program meets its stated objectives.

  • Makes home visits when necessary to meet the needs of patients.

  • Oversees professional billing process.

  • Provides and coordinates professional development activities for staff, including orientation and training.

  • Leads case consultation for difficult/complex cases across disciplines.

  • Provides specialty consultation to other health care staff.

  • Monitors and coordinates staff workloads and productivity.

  • Develops and maintains updated list of community resources for use by staff and beneficiaries.

  • Conducts and coordinates program outreach and marketing activities.

  • Communicates and collaborates with community agencies, including other MIHP providers, Child Protective Services, and Great Start Collaborative.

  • Oversees and monitors referrals, intake and follow up activities.

  • Prepares for certification reviews and submits Corrective Action Plans as required.

  • Implements continuous quality improvement: conducts chart reviews, productivity analyses, consumer satisfaction surveys, and MDHHS data reports; implements quality improvement strategies based on findings.

  • Ensures entry of MIHP data into the MDHHS database.

  • Ensures that beneficiaries are appropriately served.

  • Documents services and activities per department standards and assists with program evaluation activities to track and analyze utilization and effectiveness of services.

  • Assists in the planning, organization, implementation, evaluation and supervision of student training and provision of care and services.

  • Participates in development and ongoing review of the Quality Assurance and Performance Improvement program. Ensures priority initiatives are addressed timely.

  • Follows all policies, procedures and guidelines as outlined by Medicaid, State of Michigan, Maternal Infant Health Program, Michigan Medicine and the Medicare Conditions of Participation.

  • Participates in accreditation survey readiness activities and the survey process.

  • Participates in organization sponsored inservice training.

  • Actively engages the patient, patient representative (if any), and family, as applicable, by exhibiting respect and dignity, sharing information in a timely and unbiased manner, and welcoming and encouraging participation to achieve shared decision making.

  • Collaborates with other members of the team, including the patient, patient representative (if any), and family, to achieve the ideal patient care experience.

Other Related Duties:

  • Complies with all University of Michigan and departmental standards and expectations.

  • Supports the department?s commitment to continuous improvement activities and principles through positive and respectful contribution in all problem-solving and change processes.

  • Performs other duties as assigned.

Supervision Received From: Clinical Nursing Director

Supervision Exercised: Clinical, functional, and administrative supervision is exercised over clinical and support staff.

Physical Requirements:

The employee will be able to perform the assigned duties and responsibilities with or without reasonable accommodations:

  • Ability to walk or stand for long periods of time.

  • Ability to communicate/exchange information; read, write, speak, and comprehend English.

  • Functional visual acuity and hearing for accurate assessment and recording of patient observations and information in clinical records.

  • Manual dexterity to perform required procedure activity proficiently and safely.

  • Ability to move up to 10 pounds.

  • Ability to work in a sedentary position for long periods of time, fine motor coordination/dexterity, and may require reaching, bending, twisting, stooping, moving about.

OCCUPATIONAL HAZARD INFORMATION:

Staff members may have routine exposure to all chemical inventory, inclusive of hazardous materials, housed at all work sites for Post-Acute Care, including but not limited to the office, hospital, clinics. Chemical inventory reference materials are available to all staff in the form of hard copy and on-line Material Safety Data Sheets.

Staff may carry out assignments in, hospitals, clinics, community and Post-Acute Care office Staff may drive personal owned vehicle and walk on uneven surfaces to these various work site locations in all weather conditions on a regular basis.

Required Qualifications*

  • Current licensure as a Registered Nurse in the state of Michigan.

  • Bachelors Degree in Nursing.

  • Two years experience in a manager/supervisor role.

  • At least one year of home visiting experience.

  • At least two years of experience in healthcare

  • At least two years of experience in community-based maternal-child health programs

  • Ability to analyze, compare, and evaluate various courses of action and have the authority to make independent decisions on matters of significance, free of immediate directions, within the scope of their responsibility.

  • Highly clinical and analytical with critical thinking skills for translating and communicating the analysis into actionable information for problem-solving and decision-making at all levels within organization.

  • Strong relationship-building skills, including the demonstrated ability to build consensus amongdiverse stakeholder groups

  • Effective written and verbal communication skills

  • Ability to travel from point A to point B throughout the workday as assigned

Desired Qualifications*

  • Masters Degree in Nursing

  • Leadership experience preferred.

  • 2 years of prior working in home health.

  • Knowledge of Home Care Conditions of Participation.

  • Nurse Leadership certification or completion within one (1) year of accepting this position or when eligible to certify, whichever comes first. Certification is expected to be maintained while in this leadership role. Examples include Certified Nurse Manager and Leader (CNML) or, Nurse Executive (NE-BC). Certifications must be recognized by the Magnet Recognition Program and the American Nurses Credentialing Center (ANCC).

  • Proficient computer skills with the use of Microsoft Office Suite (Excel, PowerPoint, Word).
